This Website Privacy Statement explains how Crevi handles Personal Information (information that can identify you) that you provide to Crevi, or that is otherwise collected, on websites controlled by Crevi (together referred to as "Crevi Website(s)").

Please read this Website Privacy Statement before using Crevi Websites or submitting information to Crevi via a website. By accessing and using Crevi Website(s), you agree and consent to the collection, use and disclosure of your Personal Information as outlined in this Website Privacy Statement.

Unless otherwise specifically noted, this Website Privacy Statement does not apply to information collected from you offline, or to third-party websites to which Crevi Websites may link, but that Crevi does not control. How does Crevi collect information online?

Crevi Websites may collect information that could be potentially personally identifiable about your visits without you actively submitting such information. Unidentified information may be collected using various technologies, such as cookies and web beacons. Cookies are small text files that are transferred to your computer's hard disk by a website. Web beacons (also referred to as GIF files, pixels, or Internet tags) help Crevi understand how you navigate around Crevi Websites. As part of your use of the site, your Internet browser automatically transmits to Crevi Websites some of this unidentified information, such as the URL of the website you just visited and the browser version your computer is operating. How does Crevi secure Personal Information collected on Crevi Websites?

It is Crevi’s practice to ensure that Crevi Websites are secured using commercially reasonable technical, procedural, and administrative safeguards proportional to the sensitivity and volume of data processed.  However, the confidentiality of Personal Information transmitted over the Internet cannot be guaranteed. Crevi urges you to exercise caution when transmitting Personal Information over the Internet. Crevi cannot absolutely guarantee that unauthorized third parties will not gain access to your Personal Information; therefore, when submitting Personal Information to Crevi Websites, you must weigh both the benefits and the risks. Crevi has no control over third-party websites or their content, and Crevi's Website Privacy Statement does not apply to such websites. You should check the privacy policies of third-party websites before submitting Personal Information. This also applies to those business partners or social media services that have sharing tools on Crevi Websites. As Crevi does not control the privacy practices of these third parties, you should review the privacy settings you have set up directly with them.

How does Crevi protect the privacy of children on Crevi Websites?

No Crevi websites are intended to collect or use any Personal Information from children on Crevi Websites ("children" are individuals who have not reached the age of majority in their residential jurisdictions). Crevi does not knowingly allow children to order Crevi's products, communicate with Crevi, or use any of Crevi's online services. Should you suspect that a child of whom you are the parent or legal guardian has provided us with Personal Information, please contact Crevi using the method specified below and Crevi will work with you to address this issue.

Your rights and contact information

Where individuals have rights under laws applicable to them and upon written request, Crevi will grant individuals’ access to Personal Data that it holds about them, subject to any legal restrictions.  In addition, Crevi will permit individuals to correct, amend, or delete information that is demonstrated to be inaccurate or incomplete or to object to certain types of processing of such information or to data portability, in certain circumstances and subject to certain exceptions provided by law. Crevi may not be able to comply with a request where Personal Data has been destroyed, erased or made anonymous in accordance with Crevi’s record retention obligations and practices.  In the event that Crevi cannot provide an individual with access to his/her Personal Data, Crevi will endeavor to provide the individual with an explanation, subject to any legal or regulatory restrictions. All communications to Crevi should include the sender’s name and contact information (such as email address, phone number or mailing address), and a detailed explanation of the request. In addition, communications related to Crevi Websites should include, as applicable, the email address used for registration and the Crevi Website address on which Personal Information was provided (e.g., www.crevihealth.com). Email requests to delete, amend, or correct Personal Information should include "Deletion Request" or "Amendment/Correction Request", as applicable, in the subject line of the email. Enforcement

Crevi will investigate and attempt to resolve any complaints and disputes regarding use and disclosure of Personal Information in accordance with the principles contained in this Website Privacy Statement.

Changes to this Website Privacy Statement

Crevi reserves the right to amend this Website Privacy Statement to reflect technological advancements, legal and regulatory changes, and Crevi's business practices, subject to applicable laws. If Crevi changes its privacy practices, an updated version of this Website Privacy Statement will reflect those changes. Crevi will provide notice of such changes by updating the effective date listed on this Website Privacy Statement. Your continued interaction with Crevi, in the activities covered above, will be subject to the then-current Website Privacy Statement.
